CareMaster Medical Services has developed our Personal Support Services Department to manage clients who qualify for state-funded programs designed to provide ongoing care and assistance. We participate in multiple Medicaid programs in Georgia.

Helping Patients Stay Safely and Comfortably at Home
CareMaster Medical Services is an approved provider with the Georgia Department of Community Health (DCH) to provide a range of Medicaid Home- and Community-Based Waiver Programs that help individuals receive skilled care and daily living support — all while remaining in their own homes or community settings.
These programs are designed to reduce the need for nursing home placement by offering personalized, cost-effective, and compassionate care tailored to each client’s needs.
Structured Family Caregiving (SFC)
Supporting Family Caregivers at Home The Structured Family Caregiving (SFC) program offers financial support and professional coaching to family members or friends who care for a loved one full-time at home. Caregivers receive a daily tax-free stipend, ongoing nursing support, and care management guidance through an approved SFC agency.
To qualify, the care recipient must:

Be eligible for Georgia Medicaid

Be enrolled in the CCSP or SOURCE waiver

Require daily personal care or supervision

Live in the same home as their caregiver
Independent Care Waiver Program (ICWP)
The Independent Care Waiver Program (ICWP) helps adult Medicaid members with physical disabilities or traumatic brain injuries (TBI) live in their own homes or community settings instead of a hospital or nursing facility.

Community Care Services Program (CCSP)
Supporting Independence at Home The Community Care Services Program (CCSP) provides community-based social, health, and supportive services to eligible individuals as an alternative to nursing home placement. Services may include:

Service Options Using Resources in a Community Environment (SOURCE)
Coordinated Medical & Supportive Care The SOURCE Program (Service Options Using Resources in a Community Environment) links primary medical care and case management with long-term health services in a person’s home or community.
All SOURCE participants must be eligible for full Medicaid and meet the nursing home level of care requirement. CareMaster Medical Services is a SOURCE-approved provider, offering skilled nursing, personal care, and care coordination for individuals who require daily assistance while remaining in their homes.
